If you prefer to use your current health insurance for lab or imaging expenses, we are pleased to offer the option to send a lab requisition or imaging request form to the laboratory or imaging center of your choice.

Cancelation:

We ask that you please provide us 24-hour notice of cancellation for any of our services. A last-minute cancellation or a missed appointment creates an opening in the providers schedule that could have been utilized by another patient. Patients canceling with less than 24 hours of your scheduled appointment are subject to pay $30 cancellation fee of the services reserved. We reserve the right to charge for any appointments that are cancelled or missed without notice.
Disclaimer

For a Telemedicine Appointment, patients are required to verify their identity to the treating provider via webcam. Acceptable forms of identification include a Driver’s license, State ID, or Passport. Additionally, patients must be in the state of California, Nevada, or Florida.

 

If the provider has concerns about the patient’s condition during a virtual appointment, they may choose to cancel the appointment due to the limitations of not being able to conduct an in-person medical examination. In such cases, the patient will be offered the option to reschedule for an in-home medical/sick visit if appropriate. The initial fee or after-hours fee will be applied as a credit towards the new fee for the in-person mobile urgent care visit.

 

Furthermore, if it is determined that the patient’s condition is critical or requires urgent evaluation beyond the capabilities of telemedicine or a in home visit, the patient will be referred to their local Emergency Department for further assessment and testing.

All medical services provided by One Health Mobile necessitate follow-up with the patient’s Primary Care Provider. If the patient does not have a Primary Care Provider, we can provide a referral.
