Your Appointment Includes:
  • Medical evaluation and treatment by a licensed provider for non-emergent conditions
  • Electronic prescriptions sent to your preferred pharmacy
  • Imaging orders (X-ray, Ultrasound, CT, MRI) if medically necessary
  • Lab orders provided as clinically indicated
  • Work or school excuse note issued if appropriate
  • In-Home Point-of-Care Testing available as needed:

  • Before Booking

  • Eligibility: Please call, text, or visit our website to confirm whether your condition is appropriate for mobile urgent care services before scheduling.
  • Service Area: In-home visits are available within the San Fernando Valley. Appointments outside this area may incur an additional concierge fee, payable at time of booking.
  • After-Hours Fee: Visits requested after 6:00 PM are subject to an after-hours mobile convenience fee, which is not covered by insurance and is due at the time of booking.
  • Travel & Timing: Due to Los Angeles traffic conditions, appointment times may be delayed. We will notify you promptly if adjustments are needed.
  • Right to Refuse Service: One Health Mobile Providers reserve the right to refuse service at their discretion.

  • Scope of Services

We do not treat medical emergencies. If your condition is determined to be an emergency, you will be referred to the nearest Emergency Department.

  • Follow-Up Care: Patients are advised to follow up with their primary care provider after the visit.
  • Additional Fees: Any procedures, tests, or medications not covered by insurance are the patient’s responsibility and are due at the time of service.
  • Credit Card on File: A valid credit card is required and may be charged for:
    • Non-covered services
    • Outstanding balances after insurance processing
    • Services for uninsured patients

  • Lab Billing

  • Laboratory Services: Labs are processed through third-party laboratories (e.g., Quest Diagnostics or HealthTrackRx).
  • Billing: You may receive a bill directly from the lab or have the charges submitted to your insurance by the lab.

Cancelation Policy

  • Cancellation & No-Show Policy

  • 24-Hour Notice Required: Please provide at least 24 hours’ notice for cancellations.
  • Late Cancellations: Cancellations made with less than 24 hours’ notice are subject to a $100 cancellation fee.
  • Missed Appointments: No-show appointments without prior notice may also be charged a $100 fee.
