A provider-signed lab order is required for all lab collections.

Cancellation Policy
Please provide 24-hour notice for cancellations. Late cancellations (<24 hours) incur $100 cancellation fee.
No Refund Policy – Blood & Specimen Collection
Once a blood or specimen sample has been collected, no refunds will be issued under any circumstances.
By proceeding with the specimen collection, the patient (or legal representative) acknowledges and agrees to the following:
Services are non-refundable once collection process begins
Costs incurred at time of service are irreversible cost, including time, materials, supplies, transportation, and lab coordination, at the time of service.
No refunds for lab delays, or third-party issues
It is the patient’s responsibility to provide accurate information, valid lab order if required and to ensure they are available and prepared at appointment time
All payments are final. Thank you for understanding.
